Liva Healthcare digital diabetes programme to support 4 STPs

Liva Healthcare, a digital healthcare coaching platform, has partnered with Living Well, Taking Control, a not for profit organisation part of the National Diabetes Prevention Programme.

The partnership will support patients across 4 STPs, following securing contracts across Derbyshire, Nottingham & Nottinghamshire, Northamptonshire, and Worcestershire and Herefordshire.

Thew company will support patients with a personalised digital health coaching programme through an app, and Living Well, Taking Control will be providing group and one-to-one education support sessions.

Kristoffer From, CEO at Liva Healthcare said the solution can easily be adapted and is scalable to support each of the STPs.

He commented “The NHS is leading the charge in the global fight against diabetes. This programme is the largest of its kind and, by incorporating digital and face-to-face interventions, the NHS is demonstrating its commitment to healthcare innovation.”

The programme is designed to support long term behavioural change, and provide patients with information to support this. Sessions cover key topics such as understanding the risks of diabetes, behaviour change techniques, healthy lifestyles and mental wellbeing.

Patients can be referred to the service from their GP, which is being provided for free.
