Graphnet Health has been chosen as the county-wide integrated care record to be used across Northamptonshire, supporting 700,000 people.
It means 69 GP practices, Northampton General Hospital NHS Trust, Kettering General Hospital NHS FT, Northamptonshire Healthcare NHS FT, Northamptonshire County Council Social Care, 111 and GP out-of-hours service providers, and East Midlands Ambulance Service NHS Trust will be connected through the shared care record.
The providers are part of the Northamptonshire Health and Care Partnership, and will adopt Graphnet’s CareCentric software. It will integrate into existing core IT systems used by the provider organisations to receive a broad range of patient information at the point of care across the region.

It also means that authorised clinicians and carers will have the right information where and when it is needed, which will in turn make their jobs easier. The time they save looking for a patient’s information can be used to focus on delivering more care to our citizens.”
The next phase of the project will explore the analysis of data from across the county, to support care providers manage the population’s health and care needs, and also target interventions at relevant individuals or groups.
