Sanius Health: MyMM digital patient support programme: Supporting people with multiple myeloma (MM) between appointments
Overview: MyMM is Sanius Health’s digital patient support programme for multiple myeloma (MM), combining symptom tracking, wearable-derived insights, patient-reported outcomes, medication support and escalation pathways. It improves visibility between appointments and at home, supporting earlier action.
What happened? The programme has been developed with community-based clinical escalation support from Modality Partnership, bringing together Sanius Health’s patient-facing digital ecosystem, data infrastructure and engagement model with a strong primary care partnership. Through MyMM, patients can record key symptoms, quality of life measures, treatment-related concerns and daily health changes. Wearable-derived data can capture activity, sleep, heart rate, respiratory rate, oxygen saturation and body temperature, helping to build a more continuous view of patient wellbeing outside the clinic. Medication support and reminders help patients stay engaged with their care, while structured check-ins and engagement workflows support continuity and identify where patients may need additional guidance or escalation. The same infrastructure that supports patients in day-to-day use also creates a longitudinal real-world dataset that can improve understanding of MM burden, patient experience, treatment impact and outcomes. In an early MM cohort, Sanius captured more than 6,000 datapoints in a single month, spanning symptoms, activity, sleep and patient-reported experience.
OstomySecure Ltd: TIES® Smart Lid System
Overview: OstomySecure produces innovative solutions individuals living with a stoma. With an obsessive aim of improving the quality of life for ostomates, the i-Lid™ allows users to replace traditional stoma bags with an intelligent lid that connects to their smartphone, watch or computer.
What happened? The TIES® system with its i-Lid is equipped with sensors connected to the OstomySecure digital application for real-time monitoring and patient-clinician communication. An enhanced emptying system supports controlled, on-demand drainage. These patient-facing components are enabled by the hybrid TIES® implant. Together, they are designed to enable patient-controlled, adhesive-free stoma management without the need for a permanent stoma bag. OstomySecure has developed the technology in close collaboration with colorectal surgeons, specialist stoma nurses and most importantly, individuals living with a stoma. The company has received more than 3,000 expressions of patient interest ahead of its clinical study. The TIES® Smart Lid System incorporates a smart sliding hatch which is pressure and temperature sensing with open/closed state detection; a disposable lid; and a USB-C charging, inductive charging interface, compact bedside form factor. The mobile application is being developed for both iOS and Android platforms and is designed to provide patients with intuitive real-time monitoring, historical trend analysis, personalised alerts and device management functionality.
Leeds Community Healthcare NHS Trust (LCH): Embedding digital inclusion into everyday care to reduce health inequalities
Overview: Leeds Community Healthcare embeds digital inclusion into routine care to reduce health inequalities. Using screening, staff training, and community support hubs, patients receive practical, face‑to‑face help while non‑digital options remain. This clinically led, data‑driven approach improves access, confidence, and safety.
What happened? LCH developed a joined‑up digital inclusion pathway, embedded into routine care rather than delivered as a standalone project. Digital inclusion training is delivered through workshops, bespoke team sessions, intranet resources, and a growing digital champion network. A Digital Ability and Literacy Screening Tool was embedded into the ‘About Me’ template in SystmOne. The tool captures ability to complete core digital tasks such as video calls, email or text, and internet use; confidence levels; and patient preference. When support is needed, staff can immediately signpost patients to 100% Digital Leeds and partner organisations, access to devices and gifted data SIMs, local libraries and community digital support, and LCH digital information hub (website) and easy‑read resources. We introduced eight Digital Health Support Hubs to provide hands‑on, face‑to‑face help. Over 500 citizens have already been supported across our first hubs, with further locations planned in community venues and public events. LCH has seen improved access to care, with fewer missed or failed contacts; increased patient confidence and independence when using digital health tools; and greater staff confidence in using digital appropriately and safely.
Nurosym: Giving patients back control of their health and independence
Overview: Nurosym is a wearable neuromodulation device helping patients manage chronic symptoms, improve quality of life and regain independence. By providing evidence-based vagus nerve stimulation that can be used from home, Nurosym empowers people to take control of their health.
What happened? Created by Parasym, Nurosym is a wearable medical device that delivers non-invasive vagus nerve stimulation through the ear. With a single click, patients can access a clinically validated therapy that supports nervous system regulation and recovery. This accessibility has helped drive significant adoption, with more than 5 million treatment sessions completed globally. Research published in PLOS ONE found Nurosym increased vagus nerve activity by 61 percent and improved heart rate variability by 18 percent, key indicators of autonomic nervous system function and resilience. Additional independently funded and peer-reviewed studies have demonstrated a 78 percent reduction in inflammatory biomarkers (IL-6); 35 percent reduction in anxious thoughts; 45 percent improvement in depressive symptoms and mood; 48 percent reduction in fatigue; 30 percent improvement in sleep quality; 18 percent improvement in HRV (RMSSD); 80 percent improvement in digestive symptoms; and 45 percent reduction in postural heart rate abnormalities. The shift is no longer theoretical. Patients are managing their own recovery, with evidence behind them, supported by the same therapeutic mechanism long confined to clinical settings.
Royal Osteoporosis Society: BoneMed Online – A digital medicine support service for people on osteoporosis medicines
Overview: BoneMed Online is a free digital support service helping people with osteoporosis understand their treatment and feel confident managing it. Developed by the Royal Osteoporosis Society with academic and digital partners, it improves adherence through tailored behavioural support, reaching over 9,753 patients.
What happened? Developed by the Royal Osteoporosis Society in partnership with Keele University, Professor Rob Horne (UCL), and Personia Health, BoneMed Online addresses one of the most costly challenges in osteoporosis care: poor medication adherence. Patients complete a short digital survey that captures their prescribed medication (10 different treatments available), treatment stage, beliefs and concerns about medicines, and confidence levels. Using these responses, BoneMed Online generates a tailored support journey, including a personalised medicine summary, practical educational content, and automated follow-up email support delivered over 12 months. BoneMed Online has been integrated into major clinical systems, allowing healthcare professionals to refer patients directly within existing workflows. Since launching in May 2025, BoneMed Online has supported 9,753 people across the UK and has sent over 46,000 emails, which have been accessed over 107,700 times. Patients receive clear, relevant guidance when they are deciding whether to start treatment, managing side effects, or trying to stay on medication long term. It has been shown to improve patient confidence and engagement by 71 percent. With the generous support of The Wigoder Family Foundation.
CCSS – Centro Nacional Control de Dolor y Cuidados Paliativos: Innovation to patients
Overview: In Costa Rica, progress has been made regarding the digital transformation of palliative care services. We have developed and implemented a comprehensive digital platform that enables patients, caregivers, healthcare professionals, and support networks to access services and information through a centralized online portal.
What happened? In Costa Rica, the digital transformation of palliative care services is progressing through the Costa Rican Social Security Fund (CCSS) and the National Center for Pain Control and Palliative Care (CNCDYCP). We have developed and implemented a digital platform that enables patients, caregivers, healthcare professionals, and support networks to access services and information through a centralized online portal. Key advancements include digital referral submission for new patients, reducing administrative barriers and facilitating access to specialized care; online service requests for nursing care, home-based care, medical supplies, certificates, and other patient support services; electronic tracking mechanisms that allow users to monitor the status of their requests through unique follow-up codes; digital education resources; integration of support networks across the country, providing access to contact information and coordination points for palliative care services in different regions; and digital communication channels. Costa Rica continues to advance toward a more connected, accessible, and human-centered healthcare model, where digital transformation serves as a tool to strengthen comprehensive palliative care delivery nationwide.
