Queen Victoria Hospital NHS Foundation Trust and Altera Digital Health: Queen Victoria Hospital’s EPR go-live in partnership with Altera Digital Health: Delivering a clinically safe digital transformation
Overview: Queen Victoria Hospital NHS Foundation Trust successfully delivered its first EPR through a clinically safe, trust-wide implementation. The Altera Sunrise EPR platform, branded locally as Archie EPR went live in November 2025 with no disruption to patient care, no reduction in productivity and rapid adoption across services.
What happened? Staff engagement was supported through webinars, demonstrations, workshops, Team Talk sessions, weekly staff newsletters and visual communications. A strong internal identity, including an Archie EPR logo and “Team Pink” branding for Digital Champion visibility during go-live, supported awareness and engagement. Training was delivered through e-learning, supported by follow-up sessions and practical support. 97 percent of staff completed training, 120 Digital Champions were deployed, 24/7 on-site support was provided for the first three weeks, and a central Command Centre enabled real-time issue resolution. The trust maintained 99.5 percent performance against the four-hour standard within MIU, with no breaches and no cancellations in outpatient or theatre services. In the first months following go-live, clinicians created nearly 100,000 clinical documents and prescribed nearly 40,000 medicines, with more than 87,000 medicines administrations completed electronically through ePMA. Staff accessed other trust systems more than 430,000 times using integrated patient context links. Digital adoption increased rapidly across the organisation, with teams achieving around 90 percent digital working.
Betsi Cadwaladr University Health Board (BCUHB) and Better: Connecting care through safer medicines: BCUHB’s ePMA and Shared Medicines Record transformation
Overview: Betsi Cadwaladr University Health Board has transformed medicines management through a clinician-led ePMA programme, replacing paper processes with a safer, standardised approach. The transformation includes the creation of the Shared Medicines Record, a single, up-to-date view of medicines across care settings.
What happened? BCUHB took a clinically led, nurse-driven approach to redesign workflows from first principles. Working in close partnership with Better enabled BCUHB to influence system configuration and optimisation, ensuring alignment with Welsh policy and organisational priorities. The phased implementation involved rolling out ePMA across all inpatient wards at Wrexham Maelor Hospital following an early-adopter implementation on the Heddfan Unit. The substantial implementation included a coordinated three-day transition involving more than 550 patients being manually transcribed into the digital system. Wrexham Maelor is also the first hospital in Wales to share discharge medicines information directly into the Shared Medicine Record (SMR). This connection ensures that key medicines data, including prescriptions at discharge, is now being consolidated into Wales’s emerging consolidated medicines view. Nurses spend less time locating paper charts or clarifying prescriptions, and staff report increased confidence in medicines processes, supported by clearer information and decision support. Since go-live, more than 4,200 patients have undergone digital medicines reconciliation and almost 10,000 have recorded allergies.
Dosium: Touchdose at West London Children’s Healthcare: BNFC-governed paediatric prescribing operationalised in NHS Cerner ePMA
Overview: Touchdose, Dosium’s BNFC-anchored paediatric prescribing governance service, was delivered at Imperial in August 2024, extended to Chelsea and Westminster in late March 2025, then extended with full Cerner ePMA write-back across the WLCH partnership in May 2026.
What happened? Touchdose delivers a patient-specific, indication-based dose recommendation at the point of prescribing. The recommendation is BNFC-aligned and tailored to the individual child’s weight, age, body surface area, and condition. Touchdose has been delivered across West London Children’s Healthcare (WLCH), formed by Imperial College Healthcare (ICHT) and Chelsea and Westminster Hospital (CWFT) trusts. Touchdose went live at ICHT in August 2024. It generated patient-specific doses that prescribers copied manually into the Cerner (Oracle Health) ePMA. In March 2025, Touchdose was switched on for CWFT on the shared Cerner domain, extending the service across the WLCH partnership. In May 2026, the service was extended further with full ePMA write-back, so the BNFC-aligned dose now writes directly into Cerner at the point of prescribing across both trusts. Pre-launch workshops with paediatric pharmacy and prescribing teams surfaced the workflow friction points and shaped the priority drug list. The Imperial deployment was evaluated in a peer-reviewed before-and-after study, published in BMJ Paediatrics Open in September 2025 (Feather et al.). The study demonstrated an 83 percent reduction in the odds of a prescribing error.
