Lumeon, the provider of care pathway solutions has partnered with Medtronic a provider of integrated healthcare solutions across Europe.
The partnership will provide the technology to support Medtronic’s customer base in their transition to value-based care models.
Medtronic will utilise Lumeon’s care pathway management platform to operationalise care pathways for surgery optimisation and chronic disease management. These pathways allow providers to create more effective care coordination that reduces the cost of care delivery while minimising unwarranted variation in care.
