Shropshire, Telford and Wrekin ICS announce integrated care record system

Shropshire, Telford and Wrekin ICS has announced plans to go live with an electronic integrated care record system in summer 2021.

The ICS, working with local NHS organisations, councils and system suppliers, has launched a programme called ‘One Health and Care record’, to connect healthcare providers with patient records.

Health and social care professionals will be able to view demographic data, registered GP practice information, medications, allergies, ongoing and historic conditions, immunisations and diagnoses, test results, hospital referrals, admissions, discharges and clinics attended, and social and mental health information and care plans.

The ICS said: “Currently, local health and care services hold separate pieces of information about patients – which isn’t easily accessible between different organisations.”

During summer 2021, all GP practices within Shropshire, Telford and Wrekin will have access to One Health and Care along with:

  • Shropshire Community Health NHS Trust
  • Midlands Partnership NHS Foundation Trust
  • Shropshire Council (Social care)
  • Telford & Wrekin Council (Social care)
  • Shrewsbury and Telford Hospital NHS Trust
  • Robert Jones and Agnes Hunt Orthopaedic Hospital NHS Foundation Trust.

Andrew Roberts, Clinical Lead for the integrated care record, said: “Many regions across England have already moved to this record sharing-system, or are in process in doing so, including Staffordshire and Stoke-on-Trent.”

“Patients are often frustrated when they have to repeatedly give the same information to people from different parts of the NHS and social care. There can also be delays caused by records not being easily accessible. This new system will help put an end to that.”

To explain the new system to local people, the ICS is launching a ‘Fair Processing’ campaign this week, which will run for 12 weeks. It will promote the benefits of integrated care records, and also explain how people can opt out if they wish to.
