We’d like to say thank you to everyone who has helped us announce our winners, and of course thank you to all of our entries. We’ve had a great time reading about all your hard work and innovative efforts to help make a difference across health and care.
Now, onto the winners: we have four this year, with one for each category. So, without further ado, we present the worthy winners and highly commended runners-up of the HTN Primary Care Awards, as chosen by our judges.
Announcing the winner in the category of Primary Care Case Study of the Year is Ananya Datta, associate director of digital service delivery in primary care at South East London ICS.
Winner: X-on Health
Well done to our worthy winners X-on Health!
They carried out a proactive 12-month Access Optimisation Service (AOS) to improve patient access and levels of satisfaction at GP surgery Tudor Lodge Health Centre. X-on undertook a full telephony call flow audit and data-driven assessment, serving 11,000 patients to help tackle a rising demand for appointments and overwhelming telephone traffic during peak hours. We’d also like to congratulate Staffordshire Training Hub Differently-Abled Buddy Scheme, as our highly commended entry, for their innovative programme supporting colleagues with disabilities and neurodiversity in Primary Care.
Announcing the winner in the category of Primary Care Partnership of the Year is Lucy Glynn, community manager at X-on Health.
Winner: Vertis Health, Commissioned by NHS Herefordshire & Worcestershire and Worcestershire County Council
Congratulations to the teams at Vertis Health and NHS Herefordshire & Worcestershire and Worcestershire County Council.
This partnership provided vital health services through a collaboration with PCNs, secondary care, public health, and VCSE partners, improving access, early diagnosis, and prevention. It saw two vans deliver services up to six days per week, including NHS Health Checks, support with GP registrations, CVD/lipid and HbA1C testing, and signposting to a wide range of local services. We’d also like to congratulate our highly commended entry, Health Innovation Manchester, Greater Manchester Integrated Care Partnership and NHS Greater Manchester, whose unified vision, tailored support, and impactful initiatives, DFPC has transformed patient access to primary care across Greater Manchester.
Announcing the winner in the category of Digital Solution of the Year is Katie Lowe, head of operations at Vertis Health.
Winner: Inhealthcare and Connect Me
Huge congratulations Inhealthcare the Connect Me team!
Since 2020, Inhealthcare has worked closely with the Connect Me remote monitoring team to bring digital health care services to patients in their own homes. They have expanded a remote monitoring service for blood pressure in primary care. This service has been empowering patients to take control of their blood pressure, improving health outcomes and easing the burden on the NHS, with more than 100,000 patients using the pathway already. Our highly commended in this category is Inform Health, for the implementation of their Personal Health Record within the Midlands Partnership University NHS Foundation Trust (MPFT), which has improved patient experience through increased convenience and control in accessing its sexual health services.
Announcing the winner in the category of Improvement Initiative of the Year is Cara Afzal, programme director for digital & data at Health Innovation Manchester.
Winner: Think Healthcare – Focus Group
Congratulations to Think Healthcare – Focus Group, who take home the trophy in this category!
Their improvement initiative focused on the integration of Think Healthcare’s NHS Cloud Telephony module with a Virtual Care Navigator (VCN). This allowed for simple but effective data analysis combined with modern digital communication tools to help one practice improve patient engagement & bring big benefits to the masses. It also enabled 24/7 access to essential services, allowing patients to book, check, and cancel appointments, order repeat prescriptions, and access other vital services via phone. Congratulations are also in order for our highly commended entry, Staffordshire Training Hub GPN Foundation School which offered a structured, standardised approach to recruiting and retaining general practice nurses, addressing the need to reshape GPN recruitment culture.
