 Overview. Preston Park Community PCN in Brighton is a collaboration of five practices that have started to share resources. Working with Here, the PCN wanted to understand what they can do collectively to improve patient services by joining up their recorded data.

Why? The PCN recognised a need for higher workforce capacity and capability within the local primary care sector. The challenges come with working across multiple practices, with different clinical systems, and different ways of recording data. Therein lies the opportunity to understand variation and how best to focus resources.

What happened? Their first project focused on the development of six distinct online dashboards tailored to each individual team – the Care Home Team, First Contact Practitioner Team, Pharmacist Team, and Investment and Impact Fund Indicators. The online dashboards extract data from the GP practice systems: EMIS (using Emis X Analytics) and SystmOne. The data includes appointment utilisation and service provision. The second project involved mapping patient data to care homes in the PCN to support work around frailty; identifying vulnerable people within the system. This dashboard was then shared across all practices in the PCN to give a collective view of care home patients and their activity.

Looking ahead. Moving forwards, they are evaluating other tools and preparatory systems to enable them to spread the learning more widely and potentially across a wider network of practitioners. There is further potential to cross share success between PCN’s as a learning community.

Solve.Care

Overview. Solve.Care provides a Web3 based healthcare management platform (Care.Platform) that uses blockchain to redefine how healthcare is accessed, administered, and paid for – enabling the creation of digital health networks used to improve health outcomes through effective care coordination.

Why? Unlike traditional forms of cyber security, Care.Protocol provides a governance later for the underlying fabric of the platform; handling communication and synchronisation between participants, wallets, cards, coins, and external systems. They do not store data collectively in a traditional database or data warehouse, instead user data is placed in the control of the user using assigned Care.Nodes.

What happened? Every major enterprise platform consumes event streams to make various backend modules to communicate. Solve.Care has made sure that event streams on the platform are tokenised (hashed) and is end-to-end encrypted. All event stream payloads are encrypted using ECC (Elliptic Curve Cryptography), a highly secure key-based encryption. These end-to-end cryptographic sessions are originated at the Care.Wallet user.

Looking ahead Solve.Care continue to provide decentralised and effective security across a variety of health care providers; ensuring patient data is secure and effectively managed.

Electronic Palliative and Care Coordination System (EPaCCS)

Overview. EPaCCS provide a digital system which unifies patient data and information, allowing it to be easily accessible across sectors and ensuring the coordinated care of all patients.

Why? Patients in end-of-life care deserve to die with dignity and have their preferences known and upheld. Completeness of data shows patients’ preferences and wishes coordinated cross sector cross boundary which leads to less duplication of data, tests, medications, procedures, more time efficiencies, reduction in delayed decisions through accessible information and not having to phone for information.

What happened? EPaCCS improves the quality of care for patients, ensuring their wishes -such as preferred place of care, place of death, type of support they need – are well documented and secure, meaning they do not have to repeat themselves. This has enabled patients to have more control over their End of Life care, reduced the number of inappropriate admissions, ambulance conveys and improved symptom management. The service helps families and loves ones through the bereavement process, proving complete end-to-end care for all involved.

Looking ahead EPaCCS continues to improve their service through data reporting metrics which provide insight into their work and fuels their continued improvement; delivering personalised care which champions the needs of the patient.
