Graphnet Health and Luscii partner to launch Graphnet Remote Monitoring platform

Graphnet Health and Luscii, an OMRON Healthcare Service, have announced the launch of an advanced remote monitoring platform, Graphnet Remote Monitoring.

The platform is said to provide the “only fully integrated solution combining population health, shared care records, and remote monitoring”, to support the NHS to identify patients suitable for remote monitoring, deliver proactive out-of-hospital care, evaluate outcomes in real time and access advanced population-level insights, the suppliers shared.

Commenting on the news, Sharon Boundy, director of transformation and digital, NHS Frimley ICB, said: “We have worked with Graphnet’s remote monitoring for many years and are delighted to see this development. The merged solution will enable us to further develop our digital health ecosystem, ensuring our residents stay well with early health care intervention when needed.”

The partnership highlighted how the move comes in response to developments in preventative and digitally enabled out-of-hospital care, with forecasts suggesting an additional 500,000 patients will join remote monitoring programmes in the next two years.

On the impact, Graphnet and Luscii shared how its solutions are embedded in 20 integrated care systems, serving 17 million people through population health and shared care record services, and has delivered remote monitoring to more than 75,000 NHS patients and over 400,000 across Europe, with over 60 million patient days monitored.

The first trusts have now enrolled with the Graphnet RM platform, with more trusts scheduled to follow.

Wider trend: digital transformation

At a recent webinar for our live event series, HTN Now, we were joined by Rachel Woodington, digital care hub manager at Airedale NHS Foundation Trust, and Ruben de Neef, customer success lead at Luscii, to discuss the virtual COPD service launched at the trust including benefits, challenges and key learnings.

We were joined for a recent HTN Now panel discussion focusing on the move from reactive to proactive care by experts from across the sector, including Dan Bunstone, clinical director at Warrington ICB; Patrick Denston, PCN digital transformation and change manager at Frimley ICB; Pete Thomas, CCIO and executive director of digital development at Moorfields Eye Hospital; and Joseph Waller, director at Aire Logic. The session looked at how NHS organisations are beginning to make the transition to proactive care, the kinds of data and digital tools required to make the change, and the impacts beginning to be seen on patient care, outcomes, and operational pressures.

NHS Humber and North Yorkshire ICB has introduced a People Story Dashboard, a digital tool that offers insights into population health and workforce data, to help with “strategic planning and pathway redesign”. The ICB developed the dashboard as part of its own workforce breakthrough programme that began in 2024, with a focus on system transformation across the region. The tool aims to “revolutionise strategic planning, commissioning, and support for the three shifts outlined in the Government’s 10 Year Plan”, with access given to all staff and patients within the Humber and North Yorkshire integrated care system.
