Our health and social care system is disjointed, under-resourced and undersupported. Discharge pathways and patient journeys are inefficient putting health outcomes for patients and service users at risk, and creating immense resource pressure on care staff and commissioners. Deploying the right care technology has the power to help the system come together to enact change across both health, social care.

In this webinar we’ll demonstrate how by collaborating and co-designing within ICS’s and with stakeholders across primary and secondary care, we can positively impact the system and the people at the heart of it; users, service users and commisioners.
