This session will focus on the move from reactive to proactive care and what that really means in practice. We’ll consider some of the changes that might be required at operational and clinical levels to fully make this transition, and the challenges arising in areas like capacity when it comes to embracing truly proactive care.

With the help of a panel of experts, we’ll look at how NHS organisations are beginning to make this transition, the kinds of data and digital tools required to make the change, and the impacts beginning to be seen on patient care, outcomes, and operational pressures.

Looking ahead, we’ll consider what the future model of proactive care might look like, what that should involve, and the steps that still need to be taken to get there.
