NHS England and NHS Improvement are inviting expressions of interest from local systems to become Discharge Integration Frontrunner sites.
The aim of the programme is to develop, design and test new approaches and service models for discharging people from acute care, which could include new funding models, workforce models, or deploying new technologies.
The first phase of ‘Frontrunner Sites’ will have the objective of ensuring that more people leave acute care, have the right support in place, in a timely way.
Some of the anticipated areas of focus for the programme include: service model design, including support for designing workforce, funding, governance models and organisational design; data and analytics support, including demand and capacity planning; national support in harnessing technology; programme evaluation support; and leadership development.
NHS England and Improvement this week sent a letter to ICS executive leads, ICB chief executive designates, local authority chief executives, and directors of adult social care to invite participation.
Five to six sites are expected to be selected, with Frontrunners to be at place or ICS level. The programme is planned to take place over six to 12 months.
