This Local Digital Roadmap (LDR) takes a whole system view of priorities for digital development across the entire health system of Bath and North East Somerset. Although led by the CCG this roadmap was written with the contribution of all their local partners, covering acute, ambulance and mental health trusts, primary care, council, community, social care, end of life and palliative care organisations and with support South Central and West Commissioning Support Unit.
Recent digital achievements
In summary, key recent IM&T achievements that are contributing to the overall vision and aims of the LDR include:
- Corporate WiFi installed across all general practices
- Procurement and implementation of new clinical and operational EPR systems – TPP, EMIS and Liquid Logic (Sirona, Council and Dorothy House) and the migration of RNHRD and RUH ED onto Cerner Millennium from legacy systems.
- Establishment of a widespread, robust corporate data network across the whole organisation, including cloud based file sharing to enable mobile working (Sirona)
- Migration of Cerner Millennium EPR system from the National Programme for IT model to the Trust now having full autonomy and contractual control (RUH)
- App implementations across numerous BaNES organisations improving access to segments of the population (AWP, BaNES CCG, Council, RUH)
- 24/7 Telephone book/review/cancel of GP appointments in 24/26 practices. 7000 patients using it 14000 times in a month. 80% to book appointments, 20% booking or cancelling and so potentially reducing DNAs A data cleanse completed by all practices to ensure accuracy of information available to wider system in Adastra EPACCS
- Electronic referrals and tasking between primary care and community staff

- Paper-free improvements to corporate CCG processes, e.g. Board meetings, online expenses, record retention ICE electronic requesting software across practices and community staff enabling a view of results across regional providers for whole patient view.
- Remote monitoring using implantable loop recorder at the RUH which enables communication with primary care of abnormal results for follow up
Current initiatives
Many local initiatives are underway which are of direct relevance to the vision set out in the LDR. Some of the key ones with whole-system implications include:
- Assessment of the risk stratification tool support to proactive case finding of patients
- Further development to care pathway management software
- Roll out of Enhanced Summary Care Record to support information sharing for those most likely to present in urgent care settings.
- Increasing access to Summary Care Record in more settings
- Increase use of electronic referrals and discharge summaries between primary care and providers
- Increase use of electronic referrals between primary care and community services
- Mobile working initiatives in and out of primary care settings
- Further deployment of EPR capabilities (RUH, Sirona, AWP)
- Migration of RUH ED department to Cerner Millennium
- Development of paperless outpatients
- SWASFT currently is deploying their Electronic Care System (ECS). The system includes use of mobile devices for on-scene use by ambulance crews, capture of clinical and vital signs data, clinical decision support and handover/referral capabilities. The deployment started in 2015/16 and is due to complete by the end of 2016/17. Once the ECS is fully deployed, SWASFT will be able to report real-time on specific clinical activity and trends, via the Trust’s recently acquired data warehouse and business intelligence engine and toolset. SWASFT is planning to ensure mobile access by paramedics to the enhanced SCR, once this is widely available. It also plans to be able to send GP and specialist e-referrals.
