In rheumatology, the backlog of over 2,700 clinical letters was reduced to fewer than 200 within five months. Turnaround times improved from six months to eight days. Patients consistently reported improved consultations, with over 90 percent feeling more listened to. Documentation omissions decreased by 55 percent, coding queries dropped 15 percent, throughput tripled, completion times decreased 45 percent, and errors fell 90 percent.

Operational benefits include a 39 percent reduction in GP appointments for lifestyle issues. Multiple cohorts show reduced system utilisation with a 7 percent lower A&E attendance recorded in Yorkshire, and a breakeven ROI at 15 months.

We’d also like to extend our congratulations to Yazen for their highly commended entry for digital patient transformation for sustainable weight care.

Healthcare improvement/Operations transformation

Winner: University Hospitals of Leicester NHS Trust and Nervecentre

Congratulations to University Hospitals of Leicester NHS Trust and Nervecentre on their winning entry!

Over 280 workflows were mapped, redesigned, validated, and implemented into the new system, unlocking record sharing via shared care records and the NHS app, and paving the way for future AI and automation capabilities. Within days, digital management was in place for 18,000 appointments. Improved documentation, integrated referral triage, electronic procedure listing, and enhanced worklist visibility helped improve patient safety. Over 50 million records were migrated without incident, and national reporting resumed from day two.

A huge well done also to our highly commended entry, Health Diagnostics x Durham County Council for their Durham Health Squad, delivering mobile health and wellbeing services for the most vulnerable with Health Options®.

In peer-reviewed analysis of 91,926 remote reviews delivered in one year, remote delivery avoided 3,125,484 km of patient travel and 533,170 kg CO2e.

Congratulations also goes to Pulsetto, who earned highly commended for their work in non-invasive vagus nerve stimulation, helping people reduce stress, sleep better, and boost mental well-being.

Best AI scribe solution

Winner: Modality Partnership and Heidi Health

Congratulations to Modality Partnership and Heidi Health on their winning entry!

A 25-day pilot with 47 GPs, 2,879 consultations and 24,156 minutes of transcribed time more than halved documentation time. Modality moved rapidly to scale rollout to 190+ Modality clinicians (now >200) across GPs, nurses, pharmacists and PAs. Over 1.2 million minutes of consultations have been transcribed to date, with 17,000+ consultations auto-transcribed per month, saving 530 clinical hours each month. Heidi has delivered major, sustained efficiency gains, with a 51 percent reduction in documentation time during consultations, and a 61 percent reduction in documentation completed after hours. GPs typically reclaim 1.5–2.5 hours per day, equivalent to around 12.5 hours per week – an extra clinical session.

Our highly commended entry in this category is ScienceSoft for their AI voice scheduler for healthcare, enabling secure patient appointment management via voice – congratulations!

Best use of data

Winner: NHS Suffolk and North East Essex ICB and SISU Health

Congratulations to NHS Suffolk and North East Essex ICB and SISU Health on their winning entry!

Since May 2024, SISU Health Stations have supported SNEE ICB to deliver accessible, data-driven community blood pressure checks. SISU Health Stations provide an opportunistic, self-service health check that can be completed in approximately four minutes without the need for an appointment or clinical supervision. The programme has delivered 13,886 health checks across 19 community locations. 20.3 percent of participants recorded high blood pressure, 3,784 health checks triggered a GP call-to-action for high blood pressure and/or very high BMI, 69.5 percent of participants were overweight or obese, and 14.8 percent of participants were identified as current smokers, enabling targeted lifestyle signposting.

Congratulations also go to our highly commended in this category: CareScan: University Hospital of Derby and Burton NHS FT, for making dialysis safer and smarter with visibility and value through data.

AI solution of the year

Winner: Newcastle upon Tyne Hospitals NHS Foundation Trust and Solventum

Congratulations to Newcastle upon Tyne Hospitals NHS Foundation Trust and Solventum on their winning entry!

At Newcastle upon Tyne Hospitals NHSFT, thrombosis nurse specialists struggled to manually review 900 daily scans. In partnership with Solventum, they developed Follow-up Finder-VTE, reviewing radiology reports in real-time and generating accurate EPR patient lists. By integrating seamlessly into the EPR, the solution was designed to minimise disruption, ensure rapid adoption, and create a scalable model for replication. The results include a 3.5 times increase in hospital-acquired thrombosis (HATs) detection, a 40 percent reduction in avoidable HATs, £400,000+ efficiency savings, and two hours of nursing time released daily. A HAT dashboard now enables clinical boards to monitor and investigate potentially preventable events, supporting continuous quality improvement.

A huge well done to our highly commended entry, Ally Cares, for AI resident monitoring transforming sleep, safety and outcomes in UK care homes.

Best solution for community care

Winner: NEC Software Solutions and Surrey and Sussex Healthcare NHS Trust

Congratulations to NEC Software Solutions and Surrey and Sussex Healthcare NHS Trust on their winning entry!

Surrey and Sussex Healthcare partnered with NEC Software Solutions to tackle health inequalities in newborn hearing screening for migrant families. The team set clear objectives to increase screening of refugee babies within national timeframes, reduce “Was Not Brought” (WNB) rates, improve staff confidence and cultural competence. NEC conducted internal audits to identify patterns in WNB cases. Findings revealed that families in temporary hotel accommodation and those requiring translators were most at risk of missing appointments. Results included zero WNB appointments for refugee babies over five years, 95 percent+ screening coverage, improved staff confidence, with screeners reporting greater understanding of cultural barriers, and enhanced patient experience.

We’d also like to extend our congratulations to Alder Hey Children’s NHS Foundation Trust for their highly commended entry for their CLEO Project: Transforming community care at Alder Hey with a digital-first and patient centred approach.

Best solution for mental health

Winner: Surrey and Borders Partnership NHS Foundation Trust

Congratulations to Surrey and Borders Partnership NHS Foundation Trust on their winning entry!

ADAPT+ transformed the trust’s mental health electronic patient record to improve usability, data quality, and safety. New units, structured templates, and electronic prescribing improved data and freed clinicians’ time for direct care. Staff report significant improvements, and people using services benefit from clearer records. A major structural change was implemented with the rollout of 16 new SystmOne units, each aligned to a specific clinical service or group. These included electronic transmission of prescriptions, extended appointment diaries and improved referral handling. The number of unstructured progress notes has already halved. Electronic prescribing to community pharmacies has also seen strong uptake, increasing from fewer than 500 per month to more than 2,200 already.

Congratulations also go to Help Clinic for their highly commended entry featuring self-guided mental health support with intelligent clinical assistance.

Fife partnered with Totalmobile to deploy CareLink, a workforce solution designed specifically for community care. Within months, 90 percent of scheduling was automated. Care workers now receive their full schedule, including travel time, arrival windows, visit details, and risk information directly on their device. Managers have real-time visibility into staffing levels, activity, and capacity, supporting compliance with the Safer Staffing Act. Weekly care hours increased from 9,765 hours (May 2023) to 11,478 hours (to date 2025). That is 1,713 additional hours every single week. In the last 12 months, the project reached the milestone of 3million care visits delivered to date.
